Capitol Hill Montlake Dentistry welcomes both adults and children as young as one, and we are currently accepting new patients.
Prior to your First visit:
Prior to your visit at our facility, one of our well-trained treatment coordinators will make arrangements with you. These coordinators assist you by answering any questions in regards to our practice, philosophy, and the different types of procedures and amenities offered, as well as the details regarding your first visit. Any relevant patient forms will be provided to you. This can be done either through the mail, or may be downloaded using the links below.

For your convenience prior to coming to our office, you have the option of printing the following forms: 2 pages of Medical History, HIPPA acknowledgement, Financial Policy.
Transfer Records for New Patients: If you have been seen at another dental office, please contact them to transfer all current X-rays to our office. You can have your records transferred via email (), mail or fax them to our office (fax:206-621-7103). As a reminder, receiving records by mail usually takes about 5-7 business days, so please contact your previous dental office at least 1 week prior to your initial appointment. Below, we have provided a detailed record transfer form to ensure all necessary records are transferred in agreement with HIPPA

First Visit: What to Expect- Initial Exam
Your first visit will help us get acquainted with you and learn about your goals and desires for treatment. For all patients who have never been seen with Dr. Chin ,it is our office policy to have you come in for an initial exam. We will not provide a cleaning before an initial exam. At this appointment you will receive a full comprehensive and clinical examination to carefully assess your oral health including:
- Collect and review your medical history and current medication
- Obtain the most current Blood pressure and Heart rate measurements
- A Full Mouth (18) digital X-ray set will be taken, if not provided by your most recent dentist
- Oral cancer screening
- Evaluation of soft tissue and hard tissue- lips, tongue, upper/lower palate and oral cavity
- Evaluation of occlusion, bite, and joints
- Charting of existing restorations
- Detection of caries (cavities)
- Intraoral digital photographs
The information gathered will allow Dr. Chin to prepare a treatment plan and recommend the best options for your dental care.
Second visit- Consultation and Cleaning
Once the full mouth series of X-rays are taken, your second visit will include a one on one consultation with Dr. Chin and our patient coordinator. Dr. Chin will sit down with you and present you with a treatment plan based off of your X-rays and intra oral findings, while providing an evidence and clinically based reason for each treatment suggested. A treatment plan for your case will be provided to you along with an estimate of the costs of treatment. Our treatment coordinator will follow you through all your visits and coordinate your appointments as well as your financial payment schedules. Our patient facilitator will also help to assist you with your insurance claims and any questions you may have once you have decided to pursue further treatment.
On the same day as your consultation you will also visit with our hygienist to have a cleaning and debridement of your teeth and gum tissue to remove plaque and calculus. Our hygienist will address these certain areas with you:
- Discuss current oral hygiene and provide you with at home instructions: flossing, brushing and any additional home care products
- Take gum pocket measurements to analyze your periodontal condition
- Assign an interval of frequency for regular visits to personalize your cleaning regiment
